Marginal Zone B-cell lymphoma

چکیده
Extra-nodal MZBCL of MALT type (7% of all NHL) is an indolent disease involving most often the stomach, where it usually follows chronic gastritis due to Helicobacter pylori (HP) infection. The disease may also localize in the lung, the thyroid, the salivary gland, the orbit. Splenic MZBCL (1% of all NHL on histologic samples) usually, though not invariably, presents splenomegaly associated circulating villous lymphocytes and BM involvement. It runs an indolent course. Nodal MZBCL (2% of al NHL) is a low-grade lymphoma, frequently presenting with advanced-stage disease, but not with large masses. Early relapse after chemotherapy may be observed in some patients and survival is shorter that in MZBCL of MALT type. All subsets may transform into a high grade lymphoma.

The group of marginal zone B-cell lymphomas (MZL) comprises three different entities, extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (currently called MALT lymphoma), nodal marginal zone B-cell lymphoma (previously known as monocytoid lymphoma), and splenic marginal zone B-cell lymphoma (with or without circulating villous lymphocytes).[1-3] Primary splenic and no...
متن کاملMarginal zone lymphomas: management of nodal, splenic, and MALT NHL.
Marginal zone lymphomas are indolent B-cell lymphomas that originate from the marginal zone of B-cell follicles. For several subtypes, the initiation of disease appears to be a consequence of chronic infection and/ or inflammation. While the initial lymphoid hyperplasia is driven by physiologic antigenic stimulation, additional oncogenic events, such as chromosomal translocations leading to con...

Marginal zone lymphomas of all types (nodal, splenic, and extranodal mucosa-associated lymphoid tissue [MALT]) may show plasmacytic differentiation. Distinguishing marginal zone lymphomas from other small B-cell lymphomas with plasmacytic differentiation, especially lymphoplasmacytic lymphoma, or from plasma cell neoplasms may be challenging. Marginal zone lymphomas with plasmacytic differentia...

متن کاملCD5 expression identifies a subset of splenic marginal zone lymphomas with higher lymphocytosis: a clinico-pathological, cytogenetic and molecular study of 24 cases.
BACKGROUND Classically, splenic marginal zone B-cell lymphoma is characterized by the absence of CD5 expression. Cases of apparent splenic marginal zone B-cell lymphoma showing CD5 expression, as diagnosed by blood studies, have been described; however, in the absence of histological evidence, the correct diagnosis of these cases is controversial because of possible confusion with other CD5-pos...
متن کاملMutation analysis of the rearranged immunoglobulin heavy chain genes of marginal zone cell lymphomas indicates an origin from different marginal zone B lymphocyte subsets.
Marginal zone cell lymphoma is a recently described entity among the non-Hodgkin's lymphomas. It likely originates from the marginal zone B cells in the spleen and equivalent cells in the lymph node and extranodal tissues. Recent evidence indicates that marginal zone B cells are functionally heterogeneous and may differ with respect to the pattern of somatic hypermutation in their Ig variable g...
